residents

plan called KeyCare Advantage.
KeyCare Advantage offers specialized, coordinated care, including nurse
practitioners to help with reducing
re-hospitalizations for long-term residents.

“For our long-term care residents,
we aren’t just a place to receive care,
we are home. Making sure that we are
doing our best to meet the psychosocial needs of our residents is something
our activities departments do really
well. They plan outings, games, entertainers, and other activities based on
the residents’ interests and likes. We try
to match residents with those who have
similar interests, starting with sharing a
meal. We want everyone to be comfortable and connected to the world around
them,” Vernon says.
FutureCare’s unique skilled nursing
program and long-term care features
certified geriatric nurse practitioners,
registered nurses (RNs), licensed practical nurses (LPNs) and geriatric nursing
assistants (GNAs). Its nursing staff is
special. They know their job is to take
care of you like they would with their
own family, and they take pride in
doing just that. Together, nurse practitioners and attending physicians provide an enhanced care model for each
patient. FutureCare’s clinical providers
are extensively trained and are capable
of managing a wide array of patient
diagnosis and needs ranging from simple to complex.
Giving Back
“We are also closely tied to giving
back to the neighborhoods around our
locations,” Vernon says. “It’s just one
of the many aspects I love about this
company. We take care of and support
our neighbors and our employees. For
example, at our Sandtown location,
we host our annual Halloween festival
with hayrides, moon bounces, a DJ,
and other activities. Everything takes
place in the pumpkin patch we create,
filled with over 2,000 pumpkins, one for
each child to take home. The children
also receive a hot lunch, a trick or treat
bag, gloves and a hat, and basics they
can use.
“We also support local schools near
our centers, often partnering for intergenerational activities, backpack drives,
field days. etc. For one school that
didn’t have a playground, we provided a
complete Kaboom playground system.
Another school needed a library mural.
Other recipients include women’s shelters, homeless shelters, and winter coat
drives. Many of our centers will also

adopt a family in the winter. We figure
out what they need and go from there.
We truly believe in the philosophy of
neighbors caring for neighbors.”
Education
FutureCare encourages ongoing
training and education for its employees through tuition reimbursement programs, nursing bridge programs, and
ongoing learning platforms.
Vernon described how FutureCare
helps to train future medical professionals as well. “We partner with several colleges and universities to provide clinical
rotations and preceptor programs for
nurses, therapists and Social Workers.
Our Chief Medical Officer oversees the
Johns Hopkins University School of
Medicine’s geriatric fellowship rotation, providing student physicians with
skilled nursing experience.”
